Who Qualifies for Hospice

Those who have a life-limiting illness, as confirmed by a physician, and meet the eligibility for end-of-life care under Medicare guidelines, qualify for hospice. A physician must certify that a person’s life expectancy is six months or less. This time frame can be extended beyond the six months as long as the individual continues to meet hospice eligibility and if the symptoms are related to their terminal illness.

Hospice of the West does not discriminate against any person on the basis of race, color, national origin, disability, or age in admission, treatment, or participation in its programs, services and activities, or in employment.  For further information about this policy, contact our Corporate Compliance Officer at 602-343-6422 or TDD/State Relay at 800-842-4681.
